A cochlear implant is intended for the following individuals:

i) Individuals aged 12 months to 17 years who have bilateral sensorineural
hearing loss and who obtain little or no benefit from binaural hearing aids.
Typical preoperative hearing loss of candidates ranges from moderate to
profound hearing loss in the low speech frequencies and severe to profound
hearing loss in the high frequencies.

ii) Individuals aged 18 years and older who have bilateral, postlinguistic
sensorineural hearing loss and who achieve limited benefit from binaural
hearing aids. Typical preoperative hearing loss of adult candidates ranges
from moderate to profound hearing loss in the low speech frequencies and
severe to profound hearing loss in the high speech frequencies.

iii) Prelinguistically or perilinguistically deafened individuals aged 18 years
and older who have profound sensorineural hearing loss and who obtain
no benefit from a hearing aid. These individuals are likely to have limited
benefit from a cochlear implant.
